If your insurance provider requires a referral, then you will not be able to be seen without supplying that upon arrival at the office. Your appointment will need to be rescheduled while you secure the necessary authorization.
Patient care is our first priority at High Tide Dermatology Center.
While we do our very best to see each patient in a timely manner, there are times when a patient’s needs or concerns require more attention than anticipated.
We apologize in advance should you experience a delay, but we also commit to giving you the same level of attention and care as every one of our valued patients.
You should stop taking or using any prescription drug immediately upon experiencing a side effect.
If the reaction is not an emergency, contact us. If the side effects are creating an emergency situation, call 911 and request immediate medical attention.
Yes, for your convenience our Patient Forms are available online. Click here to access them and easily download and print all applicable forms. Fill them out and bring them with you to the office on the day of your appointment. Please note that you must also bring with you a valid insurance card, a valid photo ID such as a driver’s license and if required by your insurance company, a referral from your Primary Care Physician (unless you are a cash-paying patient).
